The computed tomographic (CT) appearance and differential diagnosis of a large subcapsular liver biloma is described. The lesion complicated an open operation for cholecystoduodenal fistula and its development distal to the site of surgery is rare. Its recognition is important for treatment planning and selection of extraperitoneal approach when surgery is contemplated.en
