Abstract
Οι ιοί Papilloma είναι οι ιοί των θηλωμάτων. Το όνομα τους προέρχεται από
την λατινική λέξη Papilla, δηλαδή θηλή ή φλύκταινα και την ελληνική κατάληξη oma
(όγκος). Οι Papilloma ιοί συγκροτούν την μεγάλη οικογένεια των Papillomaviridae. Ο
human papilloma virus (HPV) δηλαδή ιός των ανθρώπινων θηλωμάτων, ανήκει
σ'αυτή την μεγάλη οικογένεια και προσβάλει τον άνθρωπο.
Η ανακάλυψη του Papillomavirus έγινε στις αρχές του 20ου αιώνα,
συγκεκριμένα το 1933 από τον Richard Shope έπειτα από την απομόνωση του από
κουνέλια που είχαν προσβληθεί από τον ιό αυτό. Οι Papilloma είναι μικροί ιοί χωρίς
εξωτερικό περίβλημα με διάμετρο 52-55nm και δίκλωνο κυκλικό DNA μήκους
περίπου 8000 bp που κωδικοποιεί οκτώ ιϊκές πρωτεΐνες και περιέχει μια ρυθμιστική
περιοχή, την LCR. Η LCR είναι μια περιοχή μετά την κωδικοποιούσα για την L1
πρωτεΐνη περιοχή. Η μεταγραφή και ο κύκλος ζωής των ιών αυτών ρυθμίζεται από την
LCR, ένα τμήμα περίπου 900bp που περιέχει ρυθμιστικές περιοχές μεταγραφικών
παραγόντων.
Έχουν ταυτοποιηθεί πάνω από 100 γενότυποι HPV, οι οποίοι μολύνουν
δερματικούς και βλεννογόνους ιστούς του γεννητικού συστήματος. Αυτοί που
προσβάλουν τα επιθηλιακά κύτταρα του βλεννογόνου του γεννητικού συστήματος
χωρίζονται σε τρεις κατηγορίες ανάλογα με την σοβαρότητα της βλάβης που
προκαλούν και την πιθανότητα πρόκλησης καρκίνου. Στην πρώτη ομάδα υψηλής
επικινδυνότητας (High Risk) ανήκουν ιοί όπως HPV 16, 18, 31, 33, 45 που
εμπλέκονται στην ανάπτυξη καρκίνου στο ουροποιητικό σύστημα και ενδοεπιθηλιακή
νεοπλασία τραχήλου. Στην δεύτερη ομάδα ενδιάμεσης επικινδυνότητας (Middle Risk)
ανήκουν οι ιοί που ανιχνεύονται σε δυσπλασίες όπως HPV 26, 53, 66. Τέλος στην
ομάδα χαμηλής επικινδυνότητας (Low Risk) ανήκουν ιοί όπως HPV 6, 11, 40 που
συνδέονται με την πρόκληση θηλωμάτων.
Οι HPV είναι σεξουαλικώς μεταδιδόμενοι ιοί και ενοχοποιούνται για την
πρόκληση καρκίνου μιας και το DNA τους έχει ανιχνευθεί σε πέραν του 95%, σε
δείγματα από καρκινώματα τραχήλου, με κύριους ενόχους τον HPV-16 και 18 που
φαίνεται ότι επικρατούν στις τραχηλικές αλλοιώσεις σε ποσοστό μεγαλύτερο του
50%. Οι συνεχείς μολύνσεις από τα στελέχη high risk του ιού μπορεί να οδηγήσουν σε ενδοεπιθηλιακή νεοπλασία τραχήλου (CIN), αιδοίου (VIN), πέους (PIN) και πρωκτού
(ΑΙΝ).
Μιας και δεν έχει βρεθεί ακόμα κάποιος αποτελεσματικός τρόπος
καταπολέμησης των HPV η μόνη λύση προς το παρόν είναι η πρόληψη και έγκαιρη
διάγνωση τα οποία επιτυγχάνονται με το PAP test (test Παπανικολάου) το οποίο
ανιχνεύει ανωμαλίες στα τραχηλικά κύτταρα. Ο κίνδυνος ανάπτυξης καρκίνου του
τραχήλου είναι αισθητά αυξημένος σε γυναίκες άνω των 40 ετών αλλά είναι
θεραπεύσιμος αν ανιχνευθεί έγκαιρα. Για αυτούς τους λόγους είναι αναγκαία η
ανάπτυξη αξιόπιστων μεθόδων διάγνωσης, ανίχνευσης και ταυτοποίησης για την
καλύτερη κατανόηση για ερευνητικούς και θεραπευτικούς σκοπούς.
Οι κύριες μέθοδοι που χρησιμοποιούνται σήμερα είναι κυρίως η PCR και κατ
επέκταση η Nested PCR με πολύ μεγάλη ευαισθησία για να γίνει εφικτή η ανίχνευση
και η ταυτοποίηση. Αυτές είναι και οι κύριες μέθοδοι που χρησιμοποιήθηκαν και στην
παρούσα εργασία.
Ο κατεξοχήν σκοπός της εργασίας αυτής είναι η ταυτοποίηση και χρήση
αυτών των μεθόδων για την χαρτογράφηση των πολλαπλών ρυθμιστών της
μεταγραφής των HPV-16 στην γενωμική περιοχή LCR, από κλινικά δείγματα τα οποία
θεωρείται ότι είναι μολυσμένα με HPV. Στην εργασία αυτή εξετάστηκαν κάποια
δείγματα από κολπικό υγρό που παραχωρήθηκαν από το Αντικαρκινικό Νοσοκομείο
Αθηνών ʼγιος Σάββας για ερευνητικούς σκοπούς τα οποία ήταν ύποπτα για μόλυνση
από HPV. Για την επίτευξη του σκοπού της εργασίας χρειάστηκε πρώτα να
πραγματοποιηθούν έπειτα από εκχύλιση μια σειρά από PCR αρχικά για την ενίσχυση
του ιϊκού DNA και έπειτα Nested-PCR στα προϊόντα αυτά για περαιτέρω ενίσχυση
και ανίχνευση. Για τον σκοπό αυτό χρησιμοποιήθηκαν οι εκκινητές LCR-7099, LCR-
256, LCR-480 για την ενίσχυση της γενωμικής LCR περιοχής του HPV-16. Έπειτα το
ενισχυμένο γενετικό υλικό των δειγμάτων στάλθηκε για αλληλούχιση (sequencing)
και στην συνέχεια χαρτογραφήθηκαν οι ρυθμιστές μεταγραφής.
Η νουκλεοτιδική και φυλογενετική αυτή μελέτη της LCR είναι πολύ
σημαντική γιατί θα οδηγήσει στην καλύτερη κατανόηση του μηχανισμού μεταγραφής
που διέπει τους HPV και κατ επέκταση του κύκλου ζωής τους και τις
αλληλεπιδράσεις τους με τον άνθρωπο με αποτέλεσμα την καλύτερη αντιμετώπιση
τους με την ανάπτυξη κατάλληλων φαρμάκων που να στοχεύουν συγκεκριμένες
διεργασίες ή αλληλουχίες του ιού.
